Face to face and telephone consultationsPatient Home visits and Nursing Home visitsChecking and signing repeat prescriptionsAdministration - dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely mannerCover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate.About us

Clinical Responsibilities -In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashionTriage patients presenting to the surgery by telephone, online and in personMaking professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.Assessing the health care needs of patients with undifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signs of illnessIn consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and educationManage long-term conditions and patients with complex problemsRecording clear and contemporaneous consultation notes to agreed standardsCollecting data for audit purposes and complying with QOF requirementsDelivering enhanced servicesCheck, manage and process patient test resultsRefer patients to secondary care and/or relevant organisations and internally within the PCN (eg to additional roles members of the PCN team) as requiredCompiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) in line with local and national guidelinesPrescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriateIn general the post-holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basisOther Responsibilities within the Organisation -Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safetyA commitment to life-long learning and audit to ensure evidence-based best practiceContributing to evaluation/audit and clinical standard setting within the organisationContributing to the summarising of patient records and read-coding patient dataAttending training and events organised by the Practice or other agencies, where appropriate and suitable and relevant for your roleKeep up to date with current evidenced based approaches to patient care and service delivery, in line with NICE and national frameworksContribute to education and training of trainee members of staff and studentsAttending and contributing to practice and PCN meetingsPerson Specification
Qualifications
MBBS or equivalent medical degreeFull registration with GMCCertification of Completion of Training - JCPTGP/PMETBInclusion on Performers ListEvidence of a commitment to on-going personal and professional developmentPersonal Skills and Qualities
