Eden Brown Synergy is currently recruiting an experienced Qualified Social Worker (QSW) to join the Adult Social Care Review Team in Gloucestershire.This is an excellent opportunity for a registered Social Worker with significant Adult Social Care experience to undertake focused social care reviews, particularly within the Supported Accommodation Service.You will work collaboratively as part of a multidisciplinary team (MDT), reviewing existing care and support arrangements to ensure individuals receive appropriate support that meets their needs and promotes independence.Location: GloucestershirePay Rate: 35 per hourContract: Temporary - Initial 3 Months (Possible Extension)Weekly Allowance: 150 Accommodation / Commuter Mileage AllowanceKey ResponsibilitiesUndertake focused and timely social care reviews for individuals within the Supported Accommodation Service.Review existing care and support arrangements to ensure they remain appropriate and outcome-focused.Work collaboratively as part of a multidisciplinary team (MDT).Engage with service users, families, carers and relevant professionals to understand individual needs and circumstances.Identify changes in care and support requirements and recommend appropriate interventions.Apply professional judgement when reviewing care packages and making recommendations.Ensure reviews are person-centred and promote independence, choice and positive outcomes.Maintain accurate and up-to-date case records and documentation.Communicate ..... full job details .....
