Eden Brown Synergy is currently recruiting an experienced Qualified Social Worker (QSW) to join the Adult Social Care Review Team in Gloucestershire.

This is an excellent opportunity for a registered Social Worker with significant Adult Social Care experience to undertake focused social care reviews, particularly within the Supported Accommodation Service.

You will work collaboratively as part of a multidisciplinary team (MDT), reviewing existing care and support arrangements to ensure individuals receive appropriate support that meets their needs and promotes independence.

Location: Gloucestershire
Pay Rate: -35 per hour
Contract: Temporary - Initial 3 Months (Possible Extension)
Weekly Allowance: -150 Accommodation / Commuter Mileage Allowance

Key Responsibilities

• Undertake focused and timely social care reviews for individuals within the Supported Accommodation Service.

• Review existing care and support arrangements to ensure they remain appropriate and outcome-focused.

• Work collaboratively as part of a multidisciplinary team (MDT).

• Engage with service users, families, carers and relevant professionals to understand individual needs and circumstances.

• Identify changes in care and support requirements and recommend appropriate interventions.

• Apply professional judgement when reviewing care packages and making recommendations.

• Ensure reviews are person-centred and promote independence, choice and positive outcomes.

• Maintain accurate and up-to-date case records and documentation.

• Communicate effectively with partner agencies and relevant stakeholders.

• Demonstrate flexibility and adaptability in responding to service requirements.

Essential Requirements

• Qualified Social Worker with a recognised Social Work qualification.

• Current registration with Social Work England.

• Significant experience within Adult Social Care.

• Experience undertaking social care assessments and reviews.

• Ability to work effectively within a multidisciplinary team.

• Excellent communication, assessment and report-writing skills.

• Flexible and adaptable approach to work.

• Ability to work independently and manage competing priorities.

• Willingness to work agilely, with flexible on-site attendance as required.
