NHS .Leylands Medical Practice is looking for an enthusiastic and proactive Health Care Assistant to join our busy team. Leylands is a friendly, high achieving, training practice based in the Heaton and Wrose areas of Bradford and provides care to over 18,500 patients.Job Summary

Job location: 81 Leylands Lane, Heaton, Bradford, BD9 5PZ. Salary: £12.50 to £15 per hour. Contract: Permanent. Working pattern: Full-time, Part-time.Role Responsibilities

Conduct comprehensive patient medical and family histories to assist clinical decision-makingConduct UrinalysisStock managementPerform blood sample collection through venipuncture for laboratory analysisMeasure and record vital signs such as blood pressure, pulse rate, and heightAssist in the delivery of immunisation programs, including seasonal flu vaccinationsAdminister intramuscular B12 injectionsWound dressingsAid in chronic disease management, including chronic disease reviews, contributing to QOF achievementUndertake health assessments and screening proceduresAssess risks related to cardiovascular disease, diabetes, and kidney function, using established guidelinesPatient Engagement and Administrative Duties

We are a large, established training practice with 3 surgeries in the Heaton and Wrose areas of Bradford.Qualifications

EssentialQualified as a phlebotomistDesirableNVQ Level 3, Care Certificate, or equivalent qualificationExperience

EssentialExperience in a healthcare settingDesirableAt least two years of experience as a Health Care Assistant within a primary care or community health settingExperience supporting patients with complex needs, frailty, or long-term conditionsDisclosure
