Health Care Support Worker

OverviewHealth Care Support Worker role at Bradford District Care NHS Foundation Trust – Community Nursing Team. The vacancy is in the South locality of Bradford with flexibility to work across the wider Bradford area as service needs require.
Responsibilities
Deliver essential care and support to patients in their own home as part of the planned community nursing service.
Work closely with patients, families, and colleagues to ensure care is safe, effective, and person-centred.
Work as part of the Adult Community Nursing Services over a 7 day period with a shift range, typically including evenings (8–9 pm) seven days a week.
Collaborate with the wider Primary Health Care team and participate in the assessment of health needs and delivery of care to the practice population.
Engage in ongoing training and development to build a wide range of clinical skills (e.g., wound care, continence care, diabetes management including insulin administration, enteral feeding support, and medication administration).
Support apprenticeship programmes to undertake pre-registration training to become a registered nurse or Nursing Associate.
