Youth Support Worker

Are you able to support vulnerable young people in the Barry area, who have housing related support needs, including those who require support to establish and maintain a tenancy and those who are homeless? We are searching for a Youth Support Worker - for a permanent contract. What will you do? Support people aged 16 - 25 across Barry who are affected by homelessness and struggling to find, secure or maintain appropriate accommodation Undertake assessments of need in accordance with the support assessment procedure and as directed by the Projects Manager.Undertake risk assessments and risk management plans, including safety planning in accordance with policies and procedures. Work within a multi-agency settings where possible (mainly internal projects), with travel across the Vale of Glamorgan. Liaise with colleagues to provide timely, appropriate support.Work within the Supporting People Gateway framework, liaising with staff as required for case management and allocation.Deliver support and assistance to each person in accordance with the ethos, values and policies of the charity.
