We have an exciting opportunity for a Senior Care Coordinator to support in our clinical service delivery. About the role The role involves managing the care coordination service area assessment and organising the allocation of care-coordination tasks, working alongside the Adult Team Manager and Clinical Services Manager as part of the intake panel and holding a caseload offering care coordination to survivors of torture. This role ensures that service users at Freedom from Torture have a central role in planning their care planning and are supported to access the most appropriate support for their needs.
