We''re looking for an enthusiastic and organised Continuity of Care Coordinator to join our substance misuse team at HMP Brixton. This is an exciting opportunity to play a key role in strengthening the journey from prison into community treatment, building effective partnerships and helping ensure people leaving custody experience a safe and supported transition. Based primarily at HMP Brixton, you''ll work across our London services, including HMP Wormwood Scrubs and Heathrow IRC, so some travel will be required. Reasonable travel expenses will be covered. What you will be doing You''ll be at the heart of improving continuity of care across our London prison services. Your responsibilities will include: Leading monthly release audits, ensuring service users are correctly discharged on NDTMS and that any data issues are identified and resolved. Completing pre-release audits to make sure community teams are fully informed and prepared for an individual''s release. Ensuring alert forms and key information are shared with community services so that support can continue without unnecessary delays.
