We are seeking an experienced and motivated Adult Social Worker to join a dedicated Older Adults Mental Health Team. You will work with individuals aged 65+ experiencing a range of mental health conditions, supporting them to live as independently and safely as possible.

Key Responsibilities:

Undertake comprehensive Care Act assessments, reviews, and risk assessments
Develop and implement person-centred care and support plans
Work collaboratively within a multidisciplinary team, including NHS partners
Safeguard vulnerable adults in line with statutory responsibilities
Support hospital discharge planning and prevent unnecessary admissions
Engage with families, carers, and external agencies to coordinate effective care

Requirements:

Qualified Social Worker with Social Work England registration
Experience within adult social care, ideally in mental health or older adults services
Strong knowledge of relevant legislation (Care Act, Mental Health Act, MCA)
Excellent communication, assessment, and organisational skills
