Cwm Gwendraeth , we care for 59 adults, offering a range of facilities including lounges, a coffee lounge, garden spaces, a craft room, and an onsite Social Club called

Canolfan . Here, residents enjoy a hydrotherapy pool, gym, and a welcoming social space to engage in activities and events. Our team is dedicated to maintaining and improving the overall independence of the people we support through rehabilitation, stimulation, and social integration.

is our modern, purpose-built facility, offering essential and special interest training programs. It provides a great space for our team to grow professionally while supporting individuals with diverse needs. We are proud of the strong multidisciplinary team at both locations, which includes Advanced Practitioners, Occupational Therapists, Physiotherapists, Speech and Language Therapists, and more. Our aim is to maintain and where possible, improve the overall independence levels of the people we support, through rehabilitation (if appropriate), stimulation and social integration so everyone can live their best life. Cwm Gwendraeth , you’ll have the opportunity to make a real difference in the lives of the people we support. Your responsibilities will include:

Assessing Care Needs:

Evaluate each resident''s physical, psychological, and social needs and create individualized care plans, ensuring the involvement of the resident, their family, and the multidisciplinary team.

Providing High-Quality Care:

Deliver day-to-day care that promotes dignity, independence, and well-being, helping residents to live their best lives.

Leading a Multidisciplinary Team:

Supervise and support the care team, guiding and mentoring staff. You will lead shifts, organize and coordinate staff duties, and assist with staff training and development.

Monitoring and Reporting:

Continually assess care needs and evaluate the care provided. Ensure care plans and records are up-to-date and accurate, striving for the highest standards in everything you do.

Promoting Independence and Well-being:

Work with residents to support rehabilitation, stimulation, and social integration, with the goal of maintaining or improving independence and enhancing the quality of life.

Ensuring a Safe Environment:

Maintain a safe and nurturing environment for both residents and staff, adhering to all relevant policies, procedures, and regulations.

Supporting Staff Development:

Orient new staff, provide regular supervision, and support career progression through training and personal development opportunities.

What’s involvedAssess each resident''s physical, psychological and social needs and develop a care plan taking into consideration the lifestyle, gender and background of the people we care for and involve the patient, family and multi-disciplinary team.

Continuously evaluate care needs with other members of the nursing team and ensure that care plans and records are up-to-date and accurate. Continually seek to improve the care delivered.

Give a clear communication of the ageing process and how it affects each person’s health and lifestyle. Provide daily visits to assess individual needs and assist service users to maximise their comfort, dignity and independence and create the right environment which supports their physical and mental well-being. Lead a multi-disciplinary shift team, supervising staff in all respects including orientation of new staff, appraisal, training and development of care staff, organising and co-ordinating their duties, guiding, supporting, monitoring and evaluating staff performance.
