Key Responsibilities Deliver high-quality nursing care in accordance with individual care plans. Assess, monitor and evaluate residents'' health and clinical conditions. Identify changes in residents'' conditions and take appropriate action. Liaise with GPs, healthcare professionals and multidisciplinary teams. Support and supervise care staff where required.
