Belmont Recruitment are currently looking for a Care Coordinator to join Walsall Council on an initial 3-6 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday. Key Responsibilities Contribute to support planning and assessments for vulnerable adults and their carers. Support citizens to make their own decisions, exercise choice and promote independence. Assist with assessing individual needs in line with self-directed support and single assessment principles. Support citizens to promote their health, wellbeing, choice and control, with consideration of mental capacity. Undertake allocated care management duties. Communicate and engage effectively with service users, families and carers using a person-centred approach. Work closely with colleagues across the multi-disciplinary team and other community agencies. Help individuals access community resources and develop the skills and support needed to live independently.
