Senior Health and Wellbeing Practitioner - HMP Usk Prescoed and Canol Location: Cardiff Salary: £28,742.18 per annum Vacancy Type: Permanent, Full time About The Role Are you passionate about helping people make lasting change? We re looking for a Senior Health and Wellbeing Practitioner to join our Integrated Substance Misuse Team at HMP Usk, Prescoed and Canol, working full time (37.5 hours per week). This is a rewarding leadership role supporting adult males within a prison setting, combining frontline clinical work with team management and service development. You ll play a key role in supporting individuals with substance misuse and low-level mental health or emotional wellbeing needs, helping them progress through structured recovery journeys and achieve positive outcomes. What you ll be doing Line managing Health and Wellbeing Practitioners and supporting their development Managing a varied caseload and delivering 1:1 interventions and group work Carrying out assessments, care planning and appropriate referrals Working closely with healthcare teams and multi-agency partners Supporting service performance, outcomes and reporting requirements Promoting equality, diversity and inclusive practice across all work What we re looking for Experience in substance misuse, mental health or wellbeing services Strong leadership and supervisory experience Confidence delivering both group and individual interventions Ability to build effective working relationships across agencies
