About the Role:
Liquid Personnel is seeking for a dedicated qualified social worker to join it client''s busy older persons team in Nottinghamshire.
What will your responsibilities be?
In this role, you will manage a complex caseload, conducting assessments, support planning, and reviews to meet individual needs and promote independence. You will identify opportunities for Reablement services and use a Strength-Based Approach to support well-being and delay long-term care. You will contribute to Continuing Healthcare (CHC) assessments, coordinate service provision, and ensure effective outcomes. By identifying community resources and maximising individual assets, you will support preventative strategies. You will also liaise with providers, offer professional advice to individuals and carers, and undertake safeguarding responsibilities as a Safeguarding Officer.
Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving license
- Post-qualified experience
- Degree level or equivalent in Social Work
