As a GP with WellBN, you will be part of a large, multidisciplinary team dedicated to providing compassionate, community-centered care. You will work closely with allied health professionals to support patients and families, particularly those in disadvantaged communities. The role involves delivering comprehensive GP services, including consultations (in-person, telephone, and home visits), prescribing, and managing patient care. Responsibilities include assessing health needs, screening for risks, providing health education, and coordinating referrals. The postholder will make autonomous clinical decisions, maintain accurate records, contribute to audits, and perform all standard primary care duties.About us

Clinical ResponsibilitiesConduct surgery consultations, telephone consultations, and home visits, addressing queries, paperwork, and repeat prescriptions efficiently.Make professional, autonomous decisions on presenting issues, whether self-referred or referred by other healthcare workers.Assess and address patients'' health care needs, screening for disease risk factors and early signs of illness.Provide counseling and health education to promote healthier lifestyles.Manage admissions and discharges to/from caseloads and refer patients to other providers as necessary.Record consultation notes clearly and to agreed standards, maintaining high standards of documentation.Collect and analyze data for audit purposes and prescribe within the practice formulary.Perform all standard duties associated with a GP role within primary care.Other Responsibilities within the OrganisationContribute to new services and innovative care models within primary care.Maintain awareness and compliance with Practice policies, including those on prescribing, confidentiality, and health and safety.Engage in lifelong learning and audit to ensure evidence-based, best practices.Participate in delivering Enhanced Services, Quality and Outcomes Framework (QOF)/Impact and Investment Fund (IIF) targets, and other performance initiatives.Assist in the development of computer-based patient records and contribute to summarising and Read-Coding patient data.Attend training sessions and events as organised by the Practice and partner agencies.Person Specification

Experience

Qualifications and TrainingFull registration with the General Medical Council (GMC).Membership of the Royal College of General Practitioners (MRCGP) or equivalent.Evidence of commitment to Continuing Professional Development (CPD).ExperienceProven experience working as a GP in a primary care setting.Experience in managing a range of undifferentiated and undiagnosed clinical conditions.Experience with Quality and Outcomes Framework (QOF) and other quality performance initiatives.Skills and AbilitiesExcellent diagnostic and clinical skills.Ability to work autonomously while making safe, evidence-based decisions.Competent in using electronic patient record systems and clinical coding.Strong communication skills to effectively counsel, educate, and build rapport with patients and multidisciplinary teams.Demonstrated ability to contribute to service improvement and development.KnowledgeSound understanding of current NHS policies and guidelines, including clinical governance.Awareness of safeguarding practices and policies.Knowledge of prescribing protocols and formulary adherence.Personal AttributesCommitted to providing compassionate and high-quality care to all patients, including those from disadvantaged communities.Flexible and adaptable, with a willingness to contribute to new services and care models.Team-oriented with a collaborative approach to working with colleagues.Other RequirementsCommitment to diversity and inclusion, aligned with the practices ethos.Ability to work between the practices three sites as required.Eligible to work in the UK.ExperiencePrevious experience in delivering Enhanced Services or specialised clinics (e.g., hormone clinics, community outreach).Participation in audit and clinical standard-setting initiatives.KnowledgeFamiliarity with local health services and community resources.SkillsAdditional qualifications or special interests that complement the practice''s services.Disclosure and Barring Service Check
