Chosen Care Group is currently recruiting for an experienced and enthusiastic Activity Coordinator to join our team.We are looking for someone with a minimum of one year''s experience within a care setting who understands how meaningful activities can positively impact residents'' health, wellbeing and independence.The successful candidate will be creative, compassionate and organised, with the ability to develop activities tailored to residents'' individual interests, abilities, preferences and life experiences.Key ResponsibilitiesDevelop and deliver engaging activity plans based on residents'' interests, abilities and life experiences.Support residents to maintain and promote their independence and lead active, fulfilling lives.Organise social activities, including coffee mornings, games, puzzles and group sessions.Coordinate a varied calendar of indoor and outdoor activities and entertainment.Develop weekly activity plans and maintain the annual activities planner.Create an inclusive environment where residents feel comfortable participating and contributing their own ideas.Develop comprehensive Health and Wellbeing care plans.Create an individual activity profile for each resident in collaboration with staff.Build positive relationships with residents, their families and friends to understand individual lifestyle histories and preferences.Maintain accurate and up-to-date records of activities and resident participation.Regularly review activity plans with residents and ..... full job details .....
