Job Description Support residents to live more independently, with compassion at the heart of your work As a Social Worker in Adult South Complex Team, you''ll work directly with people in Kensington and Chelsea who need support to live well and safely. From your first conversation to the long-term plans you help shape, you''ll be building relationships that empower individuals, reduce risks, and make a lasting difference. What you''ll be doing: Each day will bring variety and purpose. You''ll begin by reviewing referrals, prioritising cases, and planning your visits, from meeting residents in their homes to working alongside health and care professionals across the borough. Your focus will be on assessing needs, creating person-centred care plans, and arranging the right services that enable choice, dignity, and independence. You''ll ensure that every resident you support has a voice in decisions about their life. Collaboration is key, you''ll be working closely with NHS partners, voluntary sector teams, and other professionals to deliver joined-up care. You''ll also contribute to safeguarding work, maintain clear records using digital systems, and stay current with legislation, local procedures, and best practice.
