This role involves empowering clients to manage their health, wellbeing, and independence by identifying strengths and achieving goals in partnership with subcontractors. You''ll support their engagement with community professionals, clearly explain service pathways, and co-produce group activities and peer networks. Responsibilities include promoting client safety and participation, maintaining accurate records, and ensuring confidentiality. You''ll assist with tenancy management, including rent collection, arrears, and voids, while helping clients build independent living skills.
