About the role You will: Undertake strengths-based assessments, reviews, and develop personalised care and support plans to promote independence. Work in partnership with residents, carers and professionals using a person-centred approach. Identify, assess and manage risk, including safeguarding interventions. Apply relevant legislation, guidance and policy in your practice, including safeguarding responsibilities. Work collaboratively across multi-disciplinary teams to achieve positive outcomes. Demonstrate a proactive, "can do" attitude and the ability to make sound judgements, including in emergency situations.
