This role is based at Princess Royal Hospital, with a mix of hospital team base, community visits, and some home working. Key Responsibilities: Undertake assessments and reviews in line with the Care Act. Support hospital discharges and ensure safe, timely transitions from hospital to home. Work collaboratively with health professionals and multi-disciplinary teams. Promote independence and positive outcomes for service users. Requirements: Qualified Social Worker with current SWE registration. Experience in hospital discharge or community social work. Strong understanding of the Care Act, Mental Capacity Act, and safeguarding procedures.
