Description We are in search of a Hospital Social Worker or Care Manager. To assess the needs of individuals with complex needs, taking into account levels of risk and their strengths, to protect their rights and promote their independence and interests. The successful candidate will play a vital role in delivering services that promote independence, safeguard vulnerable adults, and support the well-being of our community. Responsibility To offer professional guidance to people and their carers to enable them to make informed choices about the care services that they receive and to produce a person-centred support plan which promotes their independence, dignity, choice, self-esteem and well-being. Excellent assessment, communication, and multi-agency working skills. At least 2 years experience in a Local Authority, Health or voluntary sector setting.
