Right now, we are looking for a Community Links Advisor to play a pivotal role in our Mental Health Service in Hammersmith and Fulham. Sounds great, what will I be doing? As a Community Links Advisor, you will provide holistic, recovery-focused support to service users, helping them build independence, maintain their wellbeing, and sustain successful lives within their communities. Using person-centred approaches, SMART goals, and recovery planning tools, you will deliver one-to-one support, undertake assessments, develop risk management plans, and support individuals to achieve their recovery goals. You will work collaboratively with Community Mental Health Teams, healthcare professionals, carers, and community organisations to promote access to services, resources, and social networks. The role includes facilitating group activities, supporting community integration, advocating for recovery-focused practice, addressing wider social needs, and ensuring all support is delivered in line with safeguarding, health and safety, and best practice standards.
