Connect2Socialwork is currently seeking an experienced Senior Specialist CHC Assessor on behalf of a local authority based in North Kent. This is a pivotal role for a qualified nurse or social worker with a strong background in Continuing Healthcare (CHC) assessments and complex case management.Role Overview: The successful candidate will take the lead on Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare funding. They will manage a clinical caseload, including complex cases involving disputes and joint funding arrangements, while providing empathetic support to individuals and families throughout the assessment process.Working collaboratively with internal teams, care providers, and local authority partners, the assessor will ensure appropriate care packages are in place-regardless of CHC eligibility.
