Pay:--21.58 ltd per hour (Umbrella) or equivelant to -16.39 PAYE per hour

Job Description:

The One Referral Hub in KSS are seeking a proactive and organised individual to join our team as a PSO One Referral Hub Officer. In this role, you will be responsible for managing and coordinating referrals through the One Referral Hub, ensuring efficient access to appropriate services for individuals. You will work closely with probation practitioners, partner agencies, and service providers to streamline referral processes, monitor progress, and support positive outcomes.

Key Responsibilities:

• Manage and process referrals through the One Referral Hub, ensuring timely and accurate handling.
• Liaise with probation practitioners and partner agencies to identify suitable services for individuals based on their needs.
• Maintain clear and accurate records of referrals and outcomes using the Hubs tracking system.
• Provide guidance and support to colleagues in utilising the Hub effectively.
• Monitor and report on referral trends, identifying areas for improvement and collaboration.

Skills and Experience:

• Strong organisational and communication skills.
• Ability to build positive relationships with colleagues and partner organisations.
• Proficiency in using digital tools and maintaining accurate records.
