You will play a key role in delivering tailored care plans, promoting recovery, and enhancing the client''s independence and quality of life.Key Responsibilities for Support workerDeliver high-quality, person-centred care in the client''s homeSupport rehabilitation programmes and therapy goalsAssist with manual handling and mobility supportAdminister and monitor medication safelyWork collaboratively with healthcare professionals and therapistsMonitor progress and report changes in conditionProvide emotional and psychological support to the clientHelp preven
