We have an exciting new opportunity for a Triage and Early Interventions Officer working with older people across Herefordshire and Worcestershire to join the team in the West Mercia region working 22.5 hours a week on a fixed term basis until 1st January 2028. This is a home based role; however travel will be required across the Wes Mercia region to attend Team Days and 121 Meetings.
