Our client, Brent Council, is seeking an agency Social Worker for its Reablement service.This role will last 6 months initially but more likely up to 12 months and is paid at £32 per hour.The roles will involve the following:An integrated service that delivers fast-paced responses to hospital discharges, alongside reablement and social care support through a short-term intervention model.Our team ethos centres on promoting and enabling independence, and we maintain strong working relationships with partners and colleagues across hubs and intermediate care services.Completing Care Act assessmentsCare and Support plansMulti-agency workingWorking in a fast-paced way, taking up to 3 allocations per week and actioning them quicklyVisiting people out in the community - no telephone-based assessmentsWorking closely with Reablement providers and getting feedback from themWorking in a team which includes Occupational Therapists and OT assistantsLiaising with GPs, physiotherapists etc.Using Mosaic and benefitting from the Magic Notes to lessen admin tasksPlease let me know if you are interested?
