Housing Assessment and Support Specialist Rate of pay: £21.26 PAYE or £27.40 Umbrella per hourLocation: Stockwell SW9 (hybrid working)Job Type: Full-time, temp till late April - start ASAP Join a dedicated team as an Housing Assessment and Support Specialist, where you will play a crucial role in managing and responding to a range of concerns, referrals, and casework. This position is ideal for someone passionate about reducing risk and achieving positive outcomes for customers, with a focus on supporting vulnerable adults in receipt of social care. Day-to-day of the role: Conduct initial triage, assessments, and action planning for referrals, determining the most appropriate team and approach for case management.Provide support, advice, coaching, and practical assistance to colleagues in managing support cases, aiming to minimise risks and achieve positive outcomes.Assist in completing referrals to statutory agencies, attend multiagency meetings, and collaborate with professionals to build the skills and confidence of case owners.Maintain accurate and secure records of all casework, ensuring information is appropriately stored, shared, and acted upon.Liaise with external agencies to support effective cross-sector working and achieve positive outcomes for customers.Oversee support cases of varying complexity and risk, ensuring good coordination across the organisation and understanding of all risks.Complete ongoing risk assessments for all cases using standard and specialis
