The hospital team is a team within the Early Intervention and Prevention Hub. The hospital social work team have a crucial role in providing support and assistance to adults and their families following admission into hospital. We work collaboratively with health professionals to undertake assessments to facilitate smooth, safe and timely transitions from hospital to home or alternative care settings. We support people on their hospital journey during times of crisis to successfully navigate the complexities of healthcare processes, and connect them with essential community resources.
