We are seeking a motivated, caring, and dynamic rehabilitation nurse with experience in stroke to join our team.

Role OverviewThe Rehabilitation Nurse will work closely with the Multidisciplinary Team, including Consultants, Doctors, Matrons, Ward Managers, Staff Nurses, Healthcare Assistants, Therapists, Dieticians, Pharmacists, Psychologists, and Domestic Staff.

They will assist the Ward Manager in providing nursing care and supporting patient recovery to meet rehabilitation goals, focusing on patient needs rather than solely on the illness or condition. This role offers the opportunity to work directly with patients of neurological origin.

Key Responsibilities

Collaborate with other Allied Health Professionals and Social Care Services to maximize patient recovery.

Provide skilled, effective, and evidence-based nursing care in the Neurological Rehabilitation ward.

Identify, prioritize, and implement care plans tailored to patient needs.

Promote patient and family participation in assessments and care planning.

Ensure all care plans are complete, current, and regularly evaluated.

Conduct comprehensive risk assessments and develop management plans.

Support junior staff in care plan development and evaluation.

Participate in multidisciplinary clinical reviews.

Report new problems and concerns to the Ward Manager.

Handle body fluids and contaminated sharps safely.

Provide specialist nursing skills such as continence management, falls prevention, pressure sore management, and medication management.

Undertake nutritional assessments like MUST and support patients with nutritional needs, including PEG feeds.

About Robertson WardRobertson Ward is a 20-bed specialist neurological rehabilitation inpatient ward providing multidisciplinary care for conditions such as Stroke, MS, TBI, Parkinson''s, MND, and Spinal Cord Injuries. We serve adults aged 16-64, with older patients accepted following assessment.
