An exciting opportunity has arisen for an Assessment and Transitions Coordinator to join our specialist Aurora Provision at Merton College, supporting autistic learners and learners with complex needs to make a successful transition into college life. Working closely with the Head of Inclusive Learning and the wider Aurora team, you will play a key role in coordinating the admissions, assessment and transition process for prospective learners referred to the provision. You will ensure each learner s individual needs are fully understood and that the right support is in place to help them settle in, engage and achieve positive outcomes. As Assessment and Transitions Coordinator, you will undertake pre-admission assessments, produce clear and comprehensive assessment reports and develop person-centred transition plans. You will work closely with learners, families, local authorities, schools, colleges and other professionals to gather relevant information and ensure transitions into the provision are carefully planned and effectively supported. You will also coordinate transition and link programmes for prospective learners, contribute to open days and engagement events and, where required, deliver learning activities and support programmes that promote independence, engagement and positive outcomes.
