In this role, you will arrange and coordinate care and support services for adults (18+) who are eligible under: The Care ActThe Mental Health ActNHS Continuing Healthcare (including end-of-life care)You will support individuals transitioning from hospital or short-term care settings through the ICB, such as rehabilitation services, ensuring they receive timely and appropriate support.
