Reablement Practitioner
The starting salary for this role is 27,634 per annum (pro rata for part-time roles), working 36 hours per week. Our Reablement Service, within our Adults, Wellbeing and Health Partnerships Directorate, has undergone a year of transformation, creating new opportunities within our Reablement Practitioner roles. As a therapy-led service, Reablement supports individuals to regain independence following an illness or accident. We have opportunities available across all four of our Area Reablement Services. East 1 - Redhill, Reigate, Horley and TandridgeEast 2 - Banstead, Epsom, Mole Valley and ElmbridgeWest 1 - Guildford, Waverley, Surrey Heath and FarnhamWest 2 - Runnymede, Spelthorne and Woking We have full-time - 36 hours per week - vacancies and also part-time roles available in blocks of 4 hours between 8 hours per week and 32 hours per week. As a Reablement Practitioner you will provide practical, hands-on support in people''s homes, enabling them to work towards meaningful goals and rebuild confidence in their daily activities. This role is central to supporting integrated health and social care, preventing unnecessary hospital admissions, and helping people return home safely after a hospital stay. You will need to work flexibly across multiple bases and geographical areas as required for service resilience. This service runs every day of the year from 7am until 10pm, so flexibility around working patterns is essential. Our Offer to You26 days'' holiday, rising to 28 days
