Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Children with Disabilities (CWD) Team for Sutton Council. Duties and Responsibilities: Managing a caseload of children and young people with disabilities, ensuring their needs are assessed and met through effective care planning. Undertaking statutory assessments under the Children Act 1989 and other relevant legislation. Working collaboratively with families, schools, health professionals, and other agencies to develop and implement tailored support plans. Ensuring that children''s voices are central to assessment and planning processes. Attending and contributing to case conferences, reviews, and multi-agency meetings. Promoting independence, inclusion, and positive outcomes for children and young people with disabilities. Maintaining accurate and timely case recordings in line with statutory and council requirements.
