This role supports clients to manage their wellbeing, develop independence, and achieve personal goals through co-produced plans and community engagement. It involves liaising with professionals, maintaining accurate records, managing housing-related tasks, and ensuring service users understand and participate in their support. The role also includes promoting safety, managing resources, supporting tenancy sustainment, and working flexibly with the team to meet service objectives and contractual obligations.
