As a Mental Health Support Worker, you''ll work within our HPFT-funded services in Hertfordshire, supporting adults with lived experience of mental health challenges. You''ll help individuals build stability, develop independent living skills, and move forward in their recovery journey through a person-centred and trauma-informed approach. Working across our services in Welwyn Garden City and Cheshunt, you''ll play a vital role in empowering customers to overcome barriers, improve their wellbeing, and achieve greater independence. What you''ll do Carry out needs and risk assessments, develop and review support plans in line with policies and stakeholder requirements. Promote co-production, ensuring customers are actively involved in shaping their own support. Support customers to maximise income, access benefits, and manage their finances and household budgeting. Help customers maintain their accommodation and prepare for move-on to independent living. Accurately record all customer interactions and outcomes using electronic systems, ensuring compliance with contract and performance requirements. Liaise with colleagues, partner agencies, and stakeholders regarding safeguarding, welfare, and risk concerns.
