Older Adult Dementia Rapid Response Team - Registered NurseWe have a fabulous opportunity for a Band 5 Registered Mental Health Nurse to join our Dementia Rapid Response Team. Applications are welcome from newly qualified nurses and students in their final year who are soon to qualify. We cover the Derby City and South Derbyshire area.Responsibilities
Work closely with Cubley Court, the multidisciplinary team (MDT), and the Discharge team to ensure a smooth and safe transition for patients from the ward environment to community settings (e.g., the patient\''s own home or a care home).Maintain close links with Community Mental Health Teams, GPs, Social Care, and Care Homes.Collaborate with a wider multidisciplinary team (Clinical Lead, Occupational Therapists, Nurses, Dementia Specialist Support Workers, Psychologists, and a Consultant Psychiatrist) to manage complex cases.Conduct initial and ongoing holistic assessments of patients, gathering detailed health histories covering mental status, cognitive abilities, physical health, social circumstances, and risk factors, and collaborate with families and carers.Design individualised care plans tailored to each patient\''s needs, regularly reviewing and adjusting them as conditions change; implement risk management strategies and escalate concerns as necessary.Maintain detailed records of patient care, interventions, incidents, and outcomes with confidentiality upheld at all times.Provide regular supervision and be supported by the wider team; engage in staff development and CPD opportunities as part of a robust appraisal process.Contribute to dementia care by bridging medical interventions and person-centred support for individuals living with dementia.Qualifications and attributes
Enthusiastic, resourceful, caring, with excellent communication skills.Aware of mental health with knowledge of working with older adults.Willingness to participate in an induction and preceptorship for newly qualified staff.Clinical competence, empathy, adaptability, and teamwork are essential.Benefits
