As a Support Worker, you will play an important role in promoting independence, dignity, wellbeing and quality of life. You will work closely with the people we support, their families and other professionals to ensure they receive safe, respectful and personalised care. Key responsibilities Provide person-centred support in accordance with individual care plans. Assist with personal care, including washing, dressing and toileting, where required. Support with meals, hydration and medication in line with training and company procedures. Encourage individuals to maintain their independence and make their own choices. Provide emotional, social and practical support. Support individuals to attend appointments, activities and community services. Monitor and report changes in physical or emotional wellbeing. Maintain accurate, up-to-date and confidential records. Follow safeguarding, health and safety, infection control and confidentiality procedures. Respond appropriately to emergencies and follow relevant procedures. Complete all required training and maintain professional standards.
