You will take a person-centred, strengths-based approach, working closely with housing, health, social care and other partner agencies to help prevent repeat homelessness and achieve sustainable outcomes.The role will involve working across Northamptonshire, including street outreach sessions outside normal office hours.What you will do as a Rough Sleep Support Worker Deliver flexible and responsive street outreach, identifying and engaging with people sleeping rough and helping them access accommodation and appropriate support. Complete comprehensive risk and needs assessments, develop support plans and provide intensive, person-centred support to individuals with a range of complex needs. Support service users with areas including housing, physical and mental health, substance misuse, benefits, budgeting, debt and independent living skills. Work closely with Housing Options, Adult Social Care, health services, supported housing providers and other statutory and voluntary-sector partners.
