Your responsibilities will typically include:Conducting comprehensive assessments of individuals'' mental health, social care needs, and capacity, adhering to relevant legislation (e.g., Mental Health Act, Mental Capacity Act, Social Services and Well-being (Wales) Act).Developing robust discharge plans in collaboration with service users, families, carers, and multi-disciplinary teams (MDT) including health professionals.Identifying and coordinating access to suitable community-based services and ongoing support to promote recovery and prevent readmission.Managing complex cases, including those involving safeguarding concerns for vulnerable adults.Acting as a key point of contact for service users and their families during the hospital stay and transition to community care.Maintaining ac
