Eden Brown synergy are currently recruiting for Staffordshire County Council who are looking for Senior Social Worker - Discharge to Assess (D2A) Team Senior Social Worker - Discharge to Assess (D2A) Team Location: Staffordshire (Cannock and Stafford) Hours: 37.5 hours per week Band: 6 Start Time: 08:00 - 17:00 (with 1-hour lunch) Contract Length: 4 months - may extend if required Line Manager: Vanessa Cotton - Operational Service Delivery Lead, Discharge to Assess West We are looking to recruit an experienced Level 2 Band 6 Social Worker for the West D2A beds Discharge teams to facilitate timely discharges and prevent delayed transfers of care for all Staffordshire patients who have been placed in a Discharge to Assess bed (D2A) in private care homes as commissioned by MPFT in Cannock and Stafford. Unplanned Care West also covers the Discharge to Assess (D2A) and the individual will work closely with other Multi-Disciplinary team members including Social Workers, Social Care Assessors, Nurses, Therapists (Occupational Therapists and Physiotherapists), and MPFT Home First Team. The successful candidate will have a flexible approach and be confident to forge partnership working relationships with the Multi-Disciplinary Teams across the various hospital and community sites.
