About the role: For this role, you will be part of their outreach team, travelling to the homes of the people they support to assist them in their daily routines. You will assist with day to day tasks such as cleaning, cooking and shopping. Provide assistance out in the community to engage in social activities or work placements. Such as, going out for evening meals, bowling, shopping or meeting up with family and friends. Some of the people they support live at home with their families and some live independently, but your time and engagement helps them to thrive. As an Outreach Support Worker, you will: • Provide one-to-one support• Support individuals with daily living skills• Assist with accessing the community, attending appointments, and engaging in meaningful activities• Encourage independence, confidence, and wellbeing• Follow personalised support plans and maintain accurate records• Work collaboratively with families, professionals, and colleagues• For this role a full UK driving licence and your own car will be required. Cost of fuel to travel between services is paid at 45p per mile.
