We provide 24/7 Oncology and Haematology telephone triage service for patients receiving treatment from the Royal Shrewsbury and Telford Hospitals, and also across Lancashire, in addition to providing an urgent primary care services for patients across Powys. The Nurse Practitioner will deliver advanced, autonomous and person-centred care within a 24/7 integrated triage and care coordination service. The role spans both in-hours and out-of-hours provision and operates across three key workstreams: Oncology and Haematology Helpline; Powys Care Coordination Centre (CCC), including the Single Point of Access (SPA); and Integrated Urgent Care (IUC) services for Powys.
